The dream of new teeth in one day

Many patients with missing teeth want to go to the practice in the morning and come home the same day with the new, fixed implant dentures. Massive advertising campaigns and media reporting suggest that “fixed teeth in an hour” or “fixed teeth in a day” are possible at any time and are the standard. But this is often a dubious misdirection.

Regardless of whether a single tooth is missing, several teeth are missing, or the jaw is completely edentulous, implantological treatment achieves the most comfortable results for the patient. Implants are used as artificial tooth roots on which the dentures can be securely attached. Depending on the individual initial situation, the treatment can last a few months.

The prospect of a longer treatment period initially seems daunting to many of those affected. There is concern that you may have to live without teeth or with noticeable gaps in your teeth for weeks and months – even with soup and porridge. The chance of receiving implants and the finished dentures in one day seems very tempting. These fears are unfounded: no patient looks disfigured or goes hungry during treatment. There is also the possibility of immediate care without immediate loading. But what exactly does that mean?

Immediate care

After pulling your teeth, place the implant immediately.

The prerequisite for long-term, lasting treatment success is the safe healing of the implants: the implant forms a firm, bony connection with the jawbone, the so-called osseointegration. This process takes time, during which the implant should be protected from heavy loads and movements. If this does not happen, traumatization can occur, and, in the worst case, implant loss can result.

With immediate restoration, a temporary denture is fixed onto the implants immediately after insertion. This can be, for example, a crown on a single tooth implant or – if several teeth are missing – a bridge. The temporary restoration is individually made from tooth-colored plastic and ensures adequate aesthetics during the healing period, especially in the visible front tooth area. The shape of the temporary restoration ensures that the underlying implants are not subjected to functional strain when chewing: There is no contact with the teeth in the opposite jaw (non-occlusion). After a certain period of time, the final dentures can be attached.

Whether a concept for immediate care with a good prognosis can be implemented depends on various aspects. The quality and volume of the jawbone into which the implant is screwed are crucial. In addition, the implant must be inserted with sufficient torque (from 25 Ncm). This is the only way to achieve the required stability of the implant right from the start.

Immediate resilient dental implants

Immediate loading occurs when, after implantation, dentures are inserted with the final shape that has already been adapted to the opposing teeth. The forces generated when chewing are transmitted directly into the implant and the surrounding jawbone. Experts agree that such an immediate charge only very rarely makes sense. The German Society for Implantology in the Dental, Mouth, and Jaw Area. V. – as well as other national and international professional societies – warns that if stress is applied too early, treatment failure is inevitable. This can cause a lot of grief and suffering for the affected patient: not only do they have to cope with health restrictions such as pain, but the money invested in their own share of the treatment costs is also lost. Repairing the damage to the jaw and restoring the gap between teeth costs even more time and money.

Recommendation

Implant-supported dentures are the highest quality and most comfortable treatment option for missing teeth – provided that the treating dentist or dental implant specialist draws up a treatment plan that is precisely tailored to the patient’s individual oral situation. There are no blanket solutions in implantology; full-bodied advertising promises and enticing offers should be treated with caution. The responsible, trustworthy dentist and specialist in implantology explains to the patient comprehensively and understandably the options for immediate care and the differences in immediate loading. And he will only offer the latter as a treatment option in exceptional cases.
